Scheduled Work Hours: 9:00 am - 5:00 pm. 7 hour workday, 35 hour week. 1
hour unpaid lunch. Extra hours depending on need and discretion of program.
Start of day flexible as per discretion of the Program.

Assignment Start/End Date: Consultant as needed for period of 8 weeks. The

Agency can elect to end service at anytime during the contract period. The
period of service can be extended dependant on funding. Ideal start date Feb

SCOPE OF SERVICES

The client is seeking a qualified

consultant to assist with the development of a Varicella module in the
Automated Student Health

Record system (ASHR). ASHR is the electronic health record system of the
bureau of School

Health, deployed in all elementary and intermediate public school medical
rooms. The addition

of a varicella module will assist the Bureau of Immunizations with varicella
surveillance in

approximately 600,000 students.

The consultant under guidance from the MIS development team will develop a
varicella module

in ASHR utilizing ASP.NET. Currently data is collected by nurses faxing a
varicella form to the

BOI (bureau of Immunizations). The integration of a varicella module in ASHR
will eliminate the

need for faxes. The module will be designed to fit in to the current ASHR
data structure. The

varicella form will be used to record information per student that is
positive for varicella. The

module will integrate current student demographics information for reporting
purpose.

The consultant will target the following objectives:

- Develop varicella module

- Create print capability of completed records within ASHR

- Create aggregate reports from ASHR data using crystal reports or
SQL server reporting

services.

- Investigate use of SQL server reporting services to generate
customized reports

- Recommend varicella data structure based on business needs

- Develop data export routine to transmit data to Bureau of
Immunizations

- Investigate development of an alert system for schools with no
varicella reports

- Provide technical documentation of development work

- Knowledge of SQL Server is necessary to develop varicella reports
optimized for SQL Server.

- Knowledge of SQL Server performance tuning is required to produce
reports with the least

amount of server resources.

- In order to succeed the consultant must be able to:

- Communicate effectively with team members in a verbal and written
capacity.

- Ability to analyze business rules and convert rules into data
structure and application
